Formulated Health LLC d/b/a Daodi

1. Scope of Policy

This Financial Policy applies to all direct-to-consumer wellness services offered by Formulated Health LLC d/b/a Daodi in connection with Traditional Chinese Medicine (TCM)-informed care, including: the initial video consultation with a licensed TCM practitioner; Client evaluation and development of a personalized wellness plan; custom herbal granule formulations; monthly subscription fulfillment and recurring shipments; optional follow-up consultations or check-ins by video, telephone, or other approved remote means; all consultation, subscription, dispensing, shipping, handling, administrative, and related fees; refunds, credits, chargebacks, and adjustments; cancellations, rescheduling, and termination of services; and no-show charges and other fees for missed appointments or late cancellations.

This Policy governs the Client's payment obligations and Formulated Health LLC d/b/a Daodi's billing practices, to the extent permitted by applicable law in Maine and any other applicable law. If there is any conflict between this Policy and a separate written agreement signed by the parties, the separate written agreement controls to the extent of the conflict.

2. Fees

All fees for the services and products described in this Policy are cash pay only unless Formulated Health LLC d/b/a Daodi expressly agrees otherwise in writing. The Client is responsible for paying all charges when due, including consultation fees, subscription fees, follow-up consultation fees, shipping charges, and any other fees disclosed from time to time.

The fee for the Initial Video Consultation is the amount communicated to the Client at scheduling or before the appointment. If the Client is enrolled in Monthly Subscription Fulfillment, the Client shall pay the recurring subscription fee disclosed at enrollment or renewal, plus applicable taxes, shipping, handling, and other recurring charges.

The Client is responsible for any No-Show Charge assessed under Formulated Health LLC d/b/a Daodi's scheduling policies. Formulated Health LLC d/b/a Daodi may also charge reasonable fees for returned payments, chargebacks, collection efforts, and administrative processing to the extent permitted by law and disclosed to the Client.

Unless otherwise stated, all fees are non-refundable once the service has been provided, the product prepared or shipped, or the charge otherwise incurred.

3. Payment Methods

All amounts due are cash pay only and are not billed to or reimbursed by any governmental or private insurance plan unless Formulated Health LLC d/b/a Daodi expressly agrees otherwise in writing.

Formulated Health LLC d/b/a Daodi may accept one or more Accepted Payment Methods, which may include credit card, debit card, HSA/FSA card, electronic funds transfer, ACH debit, online payment portal, or other methods it designates.

Unless stated otherwise in writing, payment is due as follows: (a) for the Initial Video Consultation, at booking or, if permitted, upon completion; (b) for a Custom Herbal Granule Formulation or other one-time product, when the order is placed or before fulfillment; (c) for Monthly Subscription Fulfillment, on a recurring monthly basis in advance of each billing cycle; and (d) for any No-Show Charge or late cancellation fee, immediately upon assessment.

The Client authorizes Formulated Health LLC d/b/a Daodi and its payment processor to charge the designated payment method for all amounts due. If a payment is declined, reversed, disputed, or not collected, Formulated Health LLC d/b/a Daodi may suspend scheduling, withhold fulfillment, cancel subscriptions, or require an alternative payment method.

4. Subscription Terms

Subject to this Policy and applicable clinical judgment, if the Client elects recurring delivery of a Custom Herbal Granule Formulation, the Client authorizes enrollment in Monthly Subscription Fulfillment. Unless stated otherwise at enrollment, the subscription renews automatically monthly until canceled under this Policy or discontinued by Formulated Health LLC d/b/a Daodi as permitted by law.

Formulated Health LLC d/b/a Daodi will generally prepare and ship each monthly order after payment authorization, within a commercially reasonable time, subject to availability, clinical review, holidays, carrier delays, and other circumstances beyond its reasonable control.

The Client may cancel in accordance with Formulated Health LLC d/b/a Daodi's cancellation procedures, subject to any notice requirements, processing deadlines, or minimum commitment periods disclosed at enrollment or required by law.

5. Refund Policy

All amounts paid are non-refundable except as expressly stated here or as required by law.

Initial Video Consultation fees are non-refundable once the consultation is completed or the appointment time passes without timely cancellation or rescheduling.

Custom Herbal Granule Formulation fees are non-refundable once the formulation has been prepared, dispensed, shipped, or made available. Because such products are individualized, no refund is due merely because the Client changes preferences, discontinues use, or elects not to use the product after preparation or shipment.

Monthly Subscription Fulfillment amounts are generally non-refundable for any cycle already commenced, products already prepared or shipped, or services already rendered. Cancellation applies prospectively only unless Formulated Health LLC d/b/a Daodi states otherwise in writing or a refund is required by law.

Shipping, handling, and third-party pass-through charges are non-refundable once the product has shipped or the service performed, except where the product was not delivered, was materially damaged in transit, or a refund is required by law.

Billing errors, duplicate charges, unauthorized charges, and other discrepancies must be reported promptly. If Formulated Health LLC d/b/a Daodi determines a Client was incorrectly charged, it will issue a refund, credit, or other adjustment as soon as reasonably practicable.

6. Cancellation Policy

The Client may cancel an Initial Video Consultation or any follow-up consultation by providing at least twenty-four (24) hours' prior notice before the scheduled time.

The Client may cancel Monthly Subscription Fulfillment at any time by notice under Formulated Health LLC d/b/a Daodi's instructions. Unless confirmed otherwise in writing, cancellation is effective at the end of the then-current billing cycle, and the Client remains responsible for all charges incurred before the effective date. To avoid the next charge or shipment, cancellation must be received at least 5 business days before the next renewal date; otherwise it applies to the following cycle.

Cancellation does not entitle the Client to a refund for products already prepared, processed, shipped, or delivered, except as required by law or stated in this Policy.

7. Chargebacks and Payment Disputes

If the Client believes a charge is incorrect, unauthorized, duplicated, or disputed, the Client shall first contact Formulated Health LLC d/b/a Daodi at support@daodi.co in good faith with reasonable details (date, amount, and description) before initiating any chargeback, reversal, or payment dispute with a card issuer, bank, or processor, to the extent such prior notice is permitted by law.

The Client agrees not to initiate a chargeback or dispute for any amount properly due, except as required or permitted by law. If one is initiated, the Client remains responsible for all amounts not finally determined invalid.

To the fullest extent permitted by law, the Client shall reimburse Formulated Health LLC d/b/a Daodi for fees, costs, and expenses reasonably incurred from an improper, unfounded, or reversed chargeback or dispute, except where the dispute was based on a charge that was actually unauthorized or invalid.

8. Late or Failed Payments

The Client is responsible for maintaining current, valid, and sufficient payment information at all times.

If any payment method is declined, expired, invalid, revoked, or otherwise fails, Formulated Health LLC d/b/a Daodi may, to the extent permitted by law, retry the charge, request an alternative method, and/or require immediate payment. The Client remains responsible for all unpaid amounts, including bank fees, chargeback fees, collection costs, and reasonable attorneys' fees to the extent permitted by law.

If the Client has an overdue balance, Formulated Health LLC d/b/a Daodi may suspend services, postpone scheduling, withhold shipment, pause subscriptions, and/or refuse further services until the account is current.

9. Shipping and Delivery Charges

Unless otherwise stated in writing, shipping and delivery charges are as follows: (a) for Monthly Subscription Fulfillment, shipping and handling are included only if expressly identified as included at purchase or in the order summary; (b) for any one-time purchase, shipping and handling, if any, are charged separately unless Formulated Health LLC d/b/a Daodi states otherwise in writing; and (c) any taxes, duties, customs, brokerage, or similar governmental charges relating to shipment are the Client's responsibility unless prohibited by law.

Risk of loss passes to the Client upon delivery of the products to the carrier, except as required by law. The Client is responsible for providing a complete, accurate shipping address and promptly notifying Formulated Health LLC d/b/a Daodi of changes before shipment.

10. Changes to Fees and Policy

Formulated Health LLC d/b/a Daodi may revise its fees, billing practices, cancellation requirements, refund practices, payment processing procedures, and other terms of this Policy from time to time, to the extent permitted by law.

Formulated Health LLC d/b/a Daodi may provide notice by any reasonable means, including email, posting to its website or patient portal, an updated intake or billing document, or other written communication to the Client's last known contact information.

11. Contact Information for Billing Questions

For any questions regarding billing, refunds, cancellations, rescheduling, chargebacks, payment disputes, or other payment-related matters, the Client shall contact Formulated Health LLC d/b/a Daodi at support@daodi.co.

Billing inquiries should be directed promptly so Formulated Health LLC d/b/a Daodi may review and, where appropriate, resolve the matter before the Client initiates a chargeback or payment dispute.
